Health Requirements

Complete Medical Health History

A medical health history form must be on file in the Student Health Services Office prior to the student’s registration.  The form is a simple medical questionnaire that does not require a physical examination by your doctor.  The form may be found and completed online by visiting the Student Health Center's website.

Immunizations

Documentation of two doses of MMR (Measles, Mumps, and Rubella) is required for all registered students.  This requirement is waived for students born before December 31, 1956.  If immunization records are not available, medical confirmation that the student had the disease(s) or a titer (blood test), showing immunity to measles, mumps and rubella will be accepted.

The following immunizations are recommended:

• Tetanus/Diphtheria - Administered within the past 10 years
• Influenza Vaccine - Advisable for all students, especially those with asthma or other chronic illnesses  

Meningococcal Vaccination 

Students residing in on-campus housing are required by state law to receive the meningococcal vaccine or sign a waiver refusing the vaccination.  The Student Health Center will maintain records of the vaccination status of each student who resides in on-campus housing.
