2008 Health Insurance Notice

 

Lincoln University will be offering several cost and plan design options to employees for calendar year 2008 health insurance coverage through UnitedHealthCare.  The advantage of having several options is that each employee has a choice of a coverage that will best accommodate the individual needs of the employee. Lincoln University will continue to contribute up to $343.62 per month towards the employee monthly premium cost. 

 

Plan Options

 

The lowest premium cost plan is 342.12 and Lincoln University contributes 100%.  This will allow an employee no out of pocket expense for monthly premiums.  However, this option has the risk of requiring significantly more expenses in out of pocket benefits.

 

The moderate premium cost plan will have a premium cost increase of 7.2% that would require the employee to pay $24.85 of the monthly premium cost; Lincoln University will contribute $343.62, with a total employee premium of $ 368.47. This plan design will also require moderate cost increases in out of pocket benefits.

 

The highest premium cost increase plan option will have a premium cost increase of 18% that would require the employee to pay $ 61.89 of the monthly premium cost; Lincoln University will contribute $343.62 with a total employee premium of $ 405.51. This plan design will maintain the current level of out of pocket benefits.  

 

Selection Process 

 

These options are explained in more detail in the attached 2008 Health Employee Selection Form.  Each employee will be asked to identify which plan option is his/her preference. 

 

The 2008 Health Employee Selection Form will need to be completed and submitted by each employee to the Human Resource Office.  The form can be accessed at the Human Resource Office or on the HR webpage.  If an employee wants to waive the coverage please indicate that on this form.

 

For any employee wanting to participate in the Mo. Café Plan, you will need to complete and submit your cafeteria enrollment form no later December 1, 2007.   The deadline for health insurance selection submittal is December 7 if you are electing not to participate in the cafeteria plan.

 

Optional employee meetings to review the options and complete the selection form will be held on Monday November 26 at 10 a.m. and 2 p.m., Tuesday November 27 at 9 a.m.  and 2 p.m.,  The meetings will be in 300 Young Hall.  Please attend one of these meetings if possible so we can assist you in a plan selection.

 New Policy Guidelines on Tuition Waiver for Dependents

For the purpose of this policy, a dependent child is defined as:
·        one of natural birth (documented by copy of birth certificate), or
·        one of legal adoption (documented by copy of legal adoption certificate), or
·        a stepchild by marriage (documented by birth certificate and marriage license), and
·        not over the age of 24
 ·       To establish the eligibility of a dependent child, the child must complete and submit a Financial Aid          Application (FAFSA) to demonstrate dependent status.
