Health Requirement Explanation

Lincoln University requires all first-time entering and re-entering students (regardless of your age) to complete a Physical History Form before registering for classes.  This form does not require a physical examination from a physician, nor does it require a physician's signature.  Listed below are the requirements needed to fulfill the health requirement at Lincoln University: 

  1. Completed Physical Health History Form (form can be completed online at http://www.lincolnu.edu/pages/396.asp).

  2. Immunization records (baby shot record) showing 2 MMR (Mumps, Measles, and Rubella) shots.  If born before December 31, 1956, this requirement is waived.  If born after December 31, 1956, please send a copy of your immunization records.  If you have any questions about this requirement, please call the Student Health Center at (573) 681-5476.

  3. If you have had a positive TB Skin Test in the past, you should provide proof of a chest X-ray and any treatment received.

  4. If residing on campus in a residence hall, you are required to either obtain the Meninogoccal vaccination OR sign a waiver declining the vaccination.

You may call the Student Health Center, Monday-Friday, 8:00 am-5:00 pm, to check the status of your health information.  If your information is incomplete, a letter will be sent stating what is missing.  Remember to keep a copy for your records.  You will NOT be able to register for classes until you meet the health requirements.
