Photocopy a form and write in the department name, course name, etc. and submit. |

What do you mean by courses that are eligible to be taken multiple times for credit? Is this the same as "repeat" classes? |

These are not "repeat" classes where a student retakes for a better grade. These are a handful of courses, mostly Music, that can be retaken multiple times for multiple credit hours. |
