Lincoln University Student Insurance Plan, Provided by Aetna Student Health
Eligibility: All registered International students taking credit hours must be enrolled in the plan. All other registered students taking 12 or more credit hours are eligible and must be enrolled in the plan unless proof of comparable coverage is provided. All other registered students taking less than 12 credit hours and dependents of students enrolled in the plan are eligible and may participate in the plan on a voluntary basis. To enroll on a voluntary basis, you must go to www.aetnastudenthealth.com and enroll before the end of "add/drop" period of each semester (August 29, 2008 for Fall Semester 2008). ***Please Note: You may be eligible to waive the Lincoln University Health Insurance. You must take your insurance card and Student ID number online to www.aetnastudenthealth.com and complete the waiver form. If you are eligible for a waiver, an e-mail confirmation will be sent to you upon completion of the form.
Terms of Coverage: The policy for the current Fall 2008 semester becomes effective on 8/15/08 at 12:01 a.m. and expires on 1/15/09 at 12:01 a.m. Coverage for the Spring 2009 semester becomes effective 1/15/09 at 12:01 a.m. and expires on 8/15/09 at 12:01 a.m. As such, the policy for the entire current 2008-2009 school year becomes effective on 08/15/08 at 12:01 a.m. and expires on 08/15/09 at 12:01 a.m. Coverage remains in effect during holiday and vacation periods. Should an individual graduate or withdraw from the institution, the insurance shall remain in effect until the end of the period for which premium has been paid.

How to Use Your Student Health Insurance Plan
- Step 1: Understanding the guidelines of your Plan may prevent you from paying unexpected out-of-pocket fees.
- Step 2: If you become sick or injured, report to the Student Health Center (SHC) immediately so that proper treatment can be prescribed or approved. If the SHC is closed or you are away from college, you should consult a doctor and follow his or her advice.
- Step 3: Be sure to present your Student Health Insurance Card when visiting a Medical Provider outside of the SHC.
- Step 4: Confirm the Medical Provider is an In-Network Provider or a private physician. You can find an In-Network Provider on www.aetnastudenthealth.com, Find a Doctor.
- Step 5: If you are being treated by an In-Network Provider, your claim will automatically be submitted to Aetna Student Health by the Medical Provider. If you are being treated by a Non-Network Provider, ask how your claim will be filed with the Medical Provider. The Provider may ask for payment immediately.
If the Provider does not file the claim directly with Aetna Student Health, then you must file the claim by submitting an itemized bill immediately after treatment to us. Your name, social security number and school name should be written clearly on all medical bills. Always retain copies for your records.
- Step 6: Payment on filed claims for covered expenses will be mailed directly to the hospital or doctor unless proof of payment is submitted with the claim.
Once a claim is processed, an Explanation of Benefits (EOB) Statement will be mailed to you explaining the benefit paid to the Provider. The Provider will then send you a statement indicating if a balance is due.

Frequently Asked Questions (FAQ)
Q: Do I have to have insurance coverage?
R: All full-time students are required to have EITHER the Lincoln University Student Health Insurance Plan OR any other U.S. based health insurance coverage. If you are currently insured, we ask that you carefully review your health insurance policy and ensure that it meets your needs while you are on campus at Lincoln University. Go to www.aetnastudenthealth.com to waive the LU student health insurance plan.
Q: Is there a co-pay or deductible for my plan?
R: There is a $25.00 out-patient deductible or co-pay that is waived with a referral from the SHC. There is a $50.00 emergency room deductible or co-pay that is waived if admitted as an inpatient.
Q: Are vision and dental services included in the LU student insurance plan?
R: No, routine vision or dental services are not included in this plan. Aetna does offer the Vital Savings Discount progam for dental and pharmacy services as an option for students to purchase on a voluntary basis. Students do not need to be enrolled in the LU student health insurance plan in order to enroll. The cost for student only coverage is $29.00. Please refer to your student health insurance brochure for specific details or contact Aetna customer service.
Q: What do I do if I am sick and need to go to the hospital?
R: Your student health insurance plan may require you to contact Aetna Student Health when you are being admitted to the hospital on an inpatient basis. Outpatient services, including day surgery and emergency room visits, do not need to be precertified. Your specific pre-certification requirements are listed on the back side of your student health insurance indentification card.
Q: Is it my responsibility to file a medical claim?
R: If you visit an In-Network Preferred Provider, they should file a medical claim on your behalf. In the event that the provider does not submit a claim for you, see your plan brochure on how to file a medical claim.
Q: Does my health insurance plan cover treatment for substance abuse and mental illness?
R: Most insurance plans provide coverage for medically necessary treatment of substance abuse and mental illness conditions. Generally, the extent of coverage may depend on whether the treatment is provided on an inpatient or outpatient basis. Please refer to your student health brochure for specific details or call the toll-free 800# on the front of your student identification card.
Q: Does my health insurance plan cover pregnancy?
R: Most of Aetna Student Health's insurance plans provide coverage for a pregnancy the same as any other medical condition. Please refer to your student health insurance brochure for specific details or call the toll-free 800# on the front of your student identification card.
Q: Am I covered for medical treatment while studying or traveling abroad?
R: Yes, no matter where you travel as a student, you are covered under the student health insurance medical plan 24 hours worldwide.
