Center for Academic Enrichment Visitation e-Form

Instructions 

The following form has been provided for your convenience to track visitations with the student's tutor. Each time a student visits with you please fill out this form and submit it. You may print a copy for  your records.

This initiative is being done to continue to support the students who have been reinstated into the University. If you have any questions comments or concerns regarding the form please feel free to contact our office. Our information is provided below.

Procedures for use of form

1. Meet with the student.
2. After your session with the student, fill out the form summarizing the conference.
3. Submit the form electronically to the office of Academic Success and Student Retention (ASSR).
