The Success Connection Visitation e-form is now available online for you convenience. Do you require proof of tutoring/mentoring services? Let the mentor/tutor know you need documentation of your visit and they can complete a form for you electronically. They may a print a copy for your records and a copy will be electronically submitted to the Office of Enrollment Management and Student Success. If you need verification for Financial Aid you will need to request an additional copy.

The following form has been provided for your convenience to track visitations with the students you mentor/tutor. Each time student visits with you please fill out this form and submit it. You may print a copy for  your records.

This initiative is being done to continue to support our freshman population and to ensure that they are taking the necessary steps to help them succeed. If you have any questions comments or concerns regarding the form please feel free to contact our office. Our information is provided below.

Procedures for use of form

  1. Meet with the student.
  2. After your session with the student, fill out the form summarizing the conference.
  3. Submit the form electronically to the office of Enrollment Management and Student Success (EMSS).
